Assumes responsibility and accountability for clinical development and coordination of the Certified Stroke Program a. Will work with the Medical Director to develop evidence-based protocols, provide and compile outcome data and maintain stroke quality measures and NIHSS reliability. Assists with the implementation of acute care and rehabilitation of patients with stroke. Provides education to patients and/or family members as well as hospital caregivers. The RN Stroke Coordinator responsibilities include, but are not limited to, the following: 

  • Assists staff in awareness and understanding of Stroke Quality Measures and works collaboratively to continually improve patient outcomes. 
  • Performs literature reviews for the latest evidence-based care/clinical advancements in the treatment of stroke. Maintains and monitors stroke practice guidelines, order sets, and critical paths. Ensures clinical processes are updated and implemented accordingly. 
  • Performs concurrent reviews for compliance with Stroke Quality Measures and provides real-time problem solving and education with staff to correct any failures 
  • Maintains a consultative role with clinical supervisors/managers/physicians regarding stroke-specific services 
  • Assists in the management/analysis of outcomes and the development of database standards of care and clinical management 
  • Serves as the stroke clinical educator for advanced topics not provided by clinical education department 
  • Conducts discharge follow-up phone calls 
  • Helps Maintain Stroke Program Certification. Reports to the Director Metro Neurosciences and designated administrative director
