We are one of two rehab hospitals in OKC that is a joint venture between Mercy and Kindred Healthcare. It is a 36-bed rehabilitation hospital located in the southern part of the city and will serve patients recovering from strokes and other long-term illnesses or injuries and features specialized clinical programs.<\/p>\n<p><b><u>JOB SUMMARY:<\/u><\/b><\/p>\n<p>Provides patient care services that meet the psychosocial, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a registered nurse.<\/p>\n<p><b><u>ESSENTIAL FUNCTIONS:<\/u><\/b><\/p>\n<ul>\n<li>Demonstrates knowledge of medications and their correct administration based on the age and clinical condition of the patient<\/li>\n<li>Reduces the potential for medication errors by following correct medication procedures.<\/li>\n<li>Performs patient care considering needs specific to the standard of care for patient\u2019s age; includes geriatric and general inpatient rehabilitation patients.<\/li>\n<li>Under the supervision of the R.N., assists with the assessment on all patients and reassessments as per policy.\u00a0 Identifies and initiates appropriate rehabilitation nursing interventions.<\/li>\n<li>Under the supervision of the R.N., performs timely and accurate QI assessments.<\/li>\n<li>Carry out the plan of care as indicated by the patient\u2019s needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly based on changes in patient\u2019s condition and individual needs.<\/li>\n<li>Interacts professionally with patient\/family and involves patient\/family in the formation of the plan of care<\/li>\n<li>Demonstrates ability to adequately assess and reassess pain. \u00a0Utilizes appropriate pain management techniques.\u00a0 Educates the patient and family regarding pain management.<\/li>\n<li>Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical\/ health care errors.<\/li>\n<li>Formulates a teaching plan based upon the patient\u2019s individual and\u00a0 identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.<\/li>\n<li>Demonstrates ability to perform treatments and provide services according to level of licensure.<\/li>\n<li>Communicates appropriately and clearly to Registered Nurses, leadership, coworkers, physicians and the Rehabilitation Services staff.<\/li>\n<li>Participates in team conferences.<\/li>\n<li>Consults other departments as appropriate to provide for an interdisciplinary approach to the patient\u2019s needs.<\/li>\n<li>Maintains a good working relationship both within the department and with other departments.<\/li>\n<li>Provides a safe environment for patients, visitors and staff and complies with appropriate and approved safety standards.\u00a0 Manages and operates equipment safely and correctly.<\/li>\n<li>Actively participates in Continuous Quality and Performance Improvement (CQPI) activities.\u00a0 Offers suggestions for opportunities for improvements in patient care, safety, education, etc.<\/li>\n<li>Demonstrates an ability to be flexible, organized and function under stressful situations<\/li>\n<li>Other duties as assigned<\/li>\n<\/ul>\n<p><b>Qualifications<\/b><\/p>\n<ul>\n<li class=\"CxSpFirst\">Graduate of accredited licensed practical\/ vocational nurse program.<\/li>\n<li>Current state licensure as Licensed Practical\/Vocational Nurse.<\/li>\n<li>CPR\/BCLS certification.
