    POSITION PURPOSE:<\/b><\/p>\n

    ESSENTIAL JOB FUNCTIONS<\/b>\u00a0<\/b>include the following.<\/p>\n<\/div>\n

    • Provides direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the Director of Nursing and\/or a Registered Nurse designee.<\/li>\n
    • Ensures quality and safe delivery of home health services to patients\/clients and families\/caregivers.<\/li>\n
    • Implements plan of care formulated by physician and Registered Nurse.<\/li>\n
    • Provides accurate and timely documentation consistent with plan of care.<\/li>\n
    • \u00a0Assesses and provides patient\/client and family caregiver education and information pertinent to diagnosis and plan of care.<\/li>\n
    • Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards.<\/li>\n
    • Participates in coordination of home health services with Registered Nurse and clinical management.<\/li>\n
    • Assists patient in learning appropriate and executing self-care activities.<\/li>\n
    • Uses equipment and supplies effectively and efficiently.<\/li>\n
    • Documents and reports activity as required.<\/li>\n
    • Acts as an effective team member.<\/span><\/li>\n
    • Other duties as assigned.<\/span><\/li>\n<\/ul>\n

      KNOWLEDGE & EXPERIENCE<\/b>:<\/b><\/p>\n<\/div>\n

       <\/p>\n

      • Completion of state approved training program (generally in the form of a diploma, certificate, or associate degree) AND Associates Degree in Nursing or Bachelor of Science Degree for Nursing.<\/li>\n
      • RN license (pass NCLEX-PN exam)<\/li>\n
      • BLS certification<\/li>\n
      • 1-2 years recent nursing experience<\/li>\n
      • Medical knowledge, patience, compassion, kindness, critical thinking skills, knowledge of infection control practices, stamina, and physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, empathy<\/li>\n<\/ul>\n<\/div>\n
